Those who have had two doses of the Moderna or Pfizer vaccine are eligible six months after their second dose. Those who have had the Johnson and Johnson vaccine are eligible after two months.

More information on eligibility is at cdc.gov/coronavirus/2019-ncov/vaccines/recommendations/specific-groups.html.

Those with an appointment have been asked to take valid identification, their health insurance card, and vaccination card, if applicable.

A New York State consent form and patient registration can be filled out ahead of time. Those wishing to do so have been asked to send an email to [email protected] to receive those documents.
